INTRODUCTION

While I'm waiting to leave Italy for Philadelphia, sitting on my armchair in business class, I look around and realize that being here after so many trips in economy is a privilege. How many sacrifices, how much commitment before understanding what I am today, what I know and recognize in myself. Mine is a business trip. I had an idea regarding a new surgical technique and an American company is very interested in the project: it is a study on replacement cages for the vertebral disc. I like to contribute to the development of new surgical techniques and approaches to be able to bring greater benefit to the patients I meet every day in my profession. In this paper, however, I want to talk to you about other things, other than surgical techniques, microsurgery, implantable devices, surgical biomechanics. I strongly want this scientific essay to be of use to the many patients suffering from spinal problems, and my aim is to be able to reach as many people as possible with my communication.

I am surprised at the kindness and education of my neighbor and notice something in them that reminds me of myself. Today, looking back on my path full of obstacles, I can say that every time I got up, I became stronger, with a greater clarity of my resources and my abilities. This appearance of mine has allowed me to become the man and professional that I am. I am convinced that if you pursue your own path with a broader vision of what surrounds us, courage, determination and heart, you will certainly reach important goals. This is what I also try to convey to my patients during our meetings.

As you read, you will understand the meaning of the book and the reasons why so many of us suffer from back pain. Finding a solution is challenging precisely because the mechanisms underlying this pathology are complex and have ancient origins, they find explanation in our evolutionary history as human beings.
